Walkthrough:

3. Class All "Win", "Win Hard Mode", "Win the Game Under 20 Minutes", and "No Knockdown" achievements See 5. Cleanup on how to unlock Shopkeeper and Countess class - Play Hard Mode - Choose a class - Press 9 - Restart then repeat with each class - If the timer on pause menu is more than 20 minutes, press 1, it will reset the timer
4. Challenge All challenge achievements - Talk with Lord Saigen (Top NPC) - Play Challenge Mode - Press 9 - Restart then repeat with each challenge
5. Cleanup Story of Together - Play any mode - Choose any class - Enter /giveitem 19 - Press 9
Unlock Shopkeeper Class - Play Normal or Hard Mode - Press 1 - Enter /shop - Press F1 and press Q to use Potion of Nitroglycerin to destroy Shopkeeper item or use the bomb by pressing E - Press F2 when the boss health bar is visible
Leveled - Play Normal Mode - Choose a Revenge of the Countess class - Enter /noclip - No clip to lower screen - Enter /bind v spawn "The Ancient" - Press V repeatedly to spawn The Ancient, just enough not to crash your game (25-30 is a good number for me) - Press F1 and Q at the same time repeatedly - Keep spawning The Ancient and bomb until you reach level 25 - Repeat with each Revenge of the Countess class including Shopkeeper
Unlock The Countess Class - Unlock Leveled - Play Normal or Hard Mode - Choose any class - Enter the dungeon (Press F near the stair) - Press 8 - Enter /boss - Press F2 - Restart and choose the same class and game mode - Enter the dungeon (Press F near the stair) - Press 8 - Enter /boss - Press F2
Crazed Dog - Play Normal Mode - Choose Fighter or Shopkeeper - (Shopkeeper) Press F1 for coins - Press 1 - Enter /giveitem 196 - Enter /gotoroom HiddenMusic (repeat until the note start with a green note / southwest / downright) - "Attack" the note block in the correct order

NOTE: Joystick is highly recommended, diagonal attack on keyboard is almost impossible to get right. Something like Key2Joy might work.
