Paid DLCs are released roughly yearly, and occasionally there are free updates with achievements as well. Be prepared to spend some occasional time and money in maintaining your completion in the future. DLC Characters and weapons are incredibly powerful; the sooner you get them, the better.
Don't worry too much about doing the difficult achievements until you've unlocked the majority of the perks/talents, as they will help tremendously.
Regarding the exclusive level frames that require you to play on Nightmare alone; it is easier to do them on Reincarnation 1 as you can use spiritual blessings.
Co-op is fun and is required for a few achievements, but you can grind revives with Nona if needed. I would not recommend doing 'Back-to-Back' with any more than 3 players, as the co-op difficulty scaling gets absurd on the highest difficulties.
You do not have to beat Spiritual Assault even once. Each character has a win task and a grind task. Simply do the grind tasks instead of going for a win, they're fairly short and you'll save yourself a lot of effort.
